On July 23, 2026, the European Medicines Agency’s (EMA) Committee for Medicinal Products for Human Use (CHMP) adopted a positive opinion, recommending marketing authorization for the new hypercholesterolemia drug, Evlarco (obicetrapib / ezetimibe). This oral medication is recommended for the treatment of primary hypercholesterolemia or mixed dyslipidemia as an adjunct to diet. Evlarco, presented as film-coated tablets, combines obicetrapib, a cholesteryl ester transfer protein (CETP) inhibitor, and ezetimibe, a cholesterol absorption inhibitor, aiming for efficient reduction of low-density lipoprotein cholesterol (LDL-C).
Technical / Clinical Details
Evlarco provides a comprehensive approach to cholesterol management by combining two drugs with distinct mechanisms of action:
- Obicetrapib: As a CETP inhibitor, it blocks the activity of cholesteryl ester transfer protein (CETP). CETP is an enzyme involved in the transfer of cholesteryl esters from HDL cholesterol to LDL cholesterol. Its inhibition is expected to lower LDL-C levels and raise high-density lipoprotein cholesterol (HDL-C) levels.
- Ezetimibe: This drug selectively inhibits cholesterol absorption in the small intestine. It reduces the amount of dietary and biliary cholesterol absorbed into the body, thereby lowering blood cholesterol levels.
By combining these two agents, each contributing to LDL-C reduction through different pathways, Evlarco is expected to achieve potent lipid-lowering effects that may be difficult to attain with monotherapy. The formulation as film-coated tablets also considers patient convenience.

Hypercholesterolemia and mixed dyslipidemia are major risk factors for cardiovascular disease and have a high global prevalence.
