KRAS-mutant CRC therapy: LNP-mediated RNAi and 5-FU synergy

bioRxiv
Overview
Researchers developed a lipid nanoparticle (LNP)-based RNA interference strategy using a single siRNA sequence to target KRAS G12D, G12V, and G13D mutations in colorectal cancer (CRC). The approach suppresses cellular proliferation and enhances the efficacy of 5-fluorouracil (5-FU) chemotherapy. This combination demonstrates synergistic apoptosis in both monolayer cultures and 3D spheroids.
In Depth

Key Findings

The study demonstrates that a single siRNA sequence delivered via lipid nanoparticles (siRNA-LNPs) can effectively target three prevalent KRAS mutants—G12D, G12V, and G13D—to sensitize colorectal cancer (CRC) cells to chemotherapy. The combination of siRNA silencing and the fluoropyrimidine agent 5-fluorouracil (5-FU) produced synergistic effects across all three mutant models, increasing apoptosis in both monolayer cultures and 3D spheroids.

Technical Details

The siRNA-LNPs were tested across multiple cell lines to verify specificity and efficacy:

  • Targeted cells: HCT116 (G13D), SW620 (G12V), and LS180 (G12D), where the treatment reduced KRAS mRNA and protein expression.
  • Control cells: HT29 (KRAS wild-type), which showed no reduction in expression.

The treatment suppressed cellular proliferation, migration, and clonogenic survival. Transcriptomic profiling indicated that the siRNA-LNPs suppressed DNA repair programs and cell-cycle signaling, which provided the mechanistic rationale for the synergistic combination with 5-FU.

Background & Context

KRAS mutations, specifically G12D, G12V, and G13D, are major oncogenic drivers in CRC and are closely linked to therapeutic resistance. While KRAS inhibitors have been approved for the G12C subtype, these other prevalent variants have lacked approved targeted therapies, creating a significant gap in clinical treatment options for CRC patients.

Significance & Outlook

By utilizing a single siRNA sequence to address multiple mutation subtypes, this LNP-mediated RNAi strategy offers a versatile approach to overcoming resistance in KRAS-mutant CRC. The proof of concept established through the synergy with 5-FU supports further investigation into this combination as a potential therapeutic approach for patients with these specific oncogenic drivers.
