Miv-cel works by genetically modifying a patient’s own T cells to express a chimeric antigen receptor (CAR) that recognizes the CD19 antigen on the surface of B cells, thereby specifically eliminating pathogenic B cells. For patients with Stiff Person Syndrome, the median improvement in the T25FW (25-foot walk time) test, a key primary endpoint, reached 49% at 12 months, with 95% of patients maintaining a clinically meaningful response. Persistent improvements were also observed in patients with generalized myasthenia gravis. The safety profile was robust, with no high-grade cytokine release syndrome (CRS) or immune effector cell-associated neurotoxicity syndrome (ICANS) reported during the one-year follow-up. This data introduces a new therapeutic paradigm for these challenging autoimmune conditions.
Background & Context
Both Stiff Person Syndrome and generalized myasthenia gravis are rare, severe autoimmune disorders affecting neuromuscular junctions and the central nervous system, respectively. Existing treatments primarily manage symptoms but rarely achieve a definitive cure. These diseases significantly impair patients’ quality of life, often leading to progressive physical disability.
