CAR-T Cell Therapy: Solid Tumour Progress and 2026 Milestones

BIOENGINEER.ORG USA
Overview
CAR-T cell therapy has achieved its first regulatory approval for a solid tumour with Satri-cel for gastric cancer in June 2026. While blood cancer treatments are established, the field now focuses on overcoming antigen escape, inflammatory toxicity, and the immunosuppressive microenvironment of solid tumours.
In Depth

Key Findings

Satri-cel has become the first regulator-approved CAR-T-cell therapy for a solid tumour, approved by China’s National Medical Products Administration in June 2026 for CLDN18.2-positive, HER2-negative advanced gastric cancer. In the randomised Phase II CT041-ST-01 trial, Satri-cel increased median progression-free survival from 1.77 to 3.25 months compared to physician’s-choice treatment in heavily pretreated gastric and gastro-oesophageal junction cancer.

Technical Details

To combat relapse caused by antigen escape, three architectural strategies for multi-antigen targeting are being developed:

  • Pooled approaches: Administering two separate monospecific products (e.g., CD19- and CD22-directed cells).
  • Bicistronic dual-CAR T cells: A single cell carrying two independent receptors (OR-gate), which provides uniform coverage but increases genetic payload.
  • Tandem CARs: Fusing two binding domains into one receptor chain, though performance is sensitive to linker flexibility and steric hindrance.

Receptor evolution has progressed through four generations: first-generation (CD3ζ only), second-generation (adding CD28 or 4-1BB costimulation), third-generation (two costimulatory domains), and fourth-generation “armoured” CARs, which include inducible transgenes like IL-12 or IL-18 to remodel the tumour microenvironment.

Background & Context

While products like tisagenlecleucel and axicabtagene ciloleucel have treated blood cancers, solid tumours present harder challenges. Previous trials in glioblastoma (EGFRvIII) and lung tissue (HER2) highlighted issues such as rapid antigen loss, on-target off-tumour toxicity, and the presence of a dense desmoplastic stroma. The solid tumour microenvironment is further complicated by TGF-β, IL-10, and adenosine, which drive cell exhaustion via PD-1, TIM-3, and LAG-3 receptors.

Significance & Outlook

The approval of Satri-cel serves as a proof of principle for treating solid tumours, though challenges remain regarding antigen density and the frequency of grade 3 or worse adverse events. The future of the field depends on integrating receptor design with biomarker science, engineering, and health economics to reduce prohibitive costs and manufacturing bottlenecks.
