Key Findings
The phase 1 trial (BrainChild-04; NCT05768880) confirmed that repetitive intracerebroventricular (ICV) administration of quad-targeting CAR T cells is feasible and tolerable. Out of 26 enrolled patients, 100% had successful manufacturing, and 21 received at least one infusion, with a median of 8 infusions per patient (range 3-56). The median survival from the initial CAR T infusion was 8.6 months (95% CI: 5.2-10.3), with all deaths attributed to disease progression.
Technical Details
The quad-CAR T cell product targets four antigens: B7-H3, EGFR806, HER2, and IL13Rα2. The trial evaluated two cohorts: Arm A (children and young adults with DIPG/pontine DMG) and Arm B (recurrent or refractory CNS tumors); this report focuses on Arm A. Key technical observations include:
- Safety: No dose-limiting toxicities occurred during the dose-finding period. Predominant adverse events were grade 1-2, including fever (95%), headache (86%), and fatigue (81%). One dose-limiting toxicity (acoustic nerve disorder) occurred outside the evaluation window.
- Kinetics: CAR T cells were detected in the cerebrospinal fluid (CSF) of 100% of patients, and 76% showed detection across serial courses, particularly at higher dose levels.
- Immune Response: Cytokine profiling indicated dynamic immune activation within the CNS.
Background & Context
Diffuse intrinsic pontine glioma (DIPG) and diffuse midline glioma (DMG) are lethal pediatric CNS tumors with a median survival of 11 months. While CAR T cell therapies have been explored, their efficacy is often limited by immune escape and antigen heterogeneity.
Significance & Outlook
This study represents the first clinical application of quad-antigen targeting CAR T cells. By targeting four different antigens, the strategy seeks to address the tumor heterogeneity that acts as a barrier to efficacy in CNS tumors. The confirmed feasibility of manufacturing and ICV administration supports further evaluation of multi-antigen CAR T strategies for these patient populations.
