However, despite rapid development, standardization of quality control for product uniformity, purity, and potency remains urgent. Regulatory bodies like the FDA are also striving to establish stringent guidelines to prevent the misuse of products lacking sufficient scientific evidence. Currently, in the U.S., selling or using certain exosome products without FDA approval is illegal if they do not meet ‘homologous use’ criteria.

Strategic Significance and Outlook

Given their broad biological effects, IV exosome therapy holds therapeutic potential for a variety of diseases. Future research will focus on elucidating the mechanisms of exosome action in specific diseases, identifying biomarkers, and validating efficacy through large-scale randomized controlled trials. Regulatory agencies like the FDA are expected to set stringent criteria for both safety and efficacy, ensuring that only products with high-quality, evidence-based research gain approval. This approach could position exosome therapy as a safe and effective ‘cell-free’ regenerative medicine for a wide range of diseases in the future, but scientific rigor and regulatory compliance are indispensable to achieving this.
