Key Findings
Phase I results for XS411 demonstrate robust clinical improvements in motor function and quality of life. Participants showed an average improvement of more than 30 points in OFF-state MDS-UPDRS Part III scores at six months post-transplant. Additionally, patient diaries indicated an average gain of over five hours of “good ON time” per day. These gains surpassed published minimal clinically important difference (MCID) thresholds, with a trend toward greater efficacy in the higher-dose cohort.
Technical Details
XS411 is an allogeneic cell therapy consisting of induced pluripotent stem cell (iPSC)-derived dopaminergic neural progenitor cells. The therapy is designed for use without patient-specific human leukocyte antigen (HLA) matching and is administered via a standardized immunosuppressive regimen. The cells are transplanted into the bilateral posterior putamen to differentiate into functional dopaminergic neurons.
To validate these results objectively, XellSmart utilized ¹⁸F-DOPA PET molecular imaging. Post-transplant scans showed elevated specific binding ratios in the putamen, confirming that the transplanted cells survived, engrafted, and established functional dopamine synthesis and metabolic capacity.
Background and Context
The Phase I trial was a single-arm, open-label study involving nine participants with moderate-to-advanced Parkinson’s disease (Hoehn–Yahr stages 3 to 4) and a mean disease duration of approximately 10 years. The study used a 3+3 dose-escalation design. Safety objectives were met; no dose-limiting toxicities, serious adverse events, or graft-induced dyskinesia were reported, and MRI monitoring showed no tumor formation or aberrant cellular overgrowth.
Significance and Outlook
XellSmart has completed enrollment for its registrational Phase II study in China. Led by Beijing Tiantan Hospital and involving other centers such as Peking Union Medical College Hospital, the trial employs a Prospective Randomized Open-label Blinded Endpoint (PROBE) design. This methodology uses randomized grouping and blinded endpoint evaluation to mitigate investigator bias and provide rigorous evidence for subsequent confirmatory clinical trials.
