XellSmart XS411: Phase I iPSC Parkinson’s Therapy Results

Longevity.Technology USA
Overview
XellSmart’s off-the-shelf cell therapy XS411 increased “good ON time” by an average of over five hours per day in nine Phase I patients. The treatment utilizes iPSC-derived dopamine neurons to target the putamen, aiming for a scalable, non-personalized manufacturing model.
In Depth

Key Findings

In a Phase I trial involving nine patients who had lived with Parkinson’s for approximately ten years, XellSmart’s XS411 therapy resulted in an average gain of more than five extra hours of “good ON time” per day at six months. This metric refers to periods where symptoms are controlled without troublesome involuntary movements. Additionally, OFF-state MDS-UPDRS Part III scores improved by more than 30 points on average, with all participants exceeding thresholds for clinically meaningful change. Results from the PDQ-39 patient questionnaire mirrored these improvements.

Technical Details

XS411 is an allogeneic, off-the-shelf therapy derived from induced pluripotent stem cells (iPSCs) coaxed into dopamine-making neurons. Surgeons inject these cells into the putamen on both sides of the brain. Unlike many donor-cell treatments that require close HLA immune matching, XS411 is designed to work without a match, utilizing a standardized immunosuppressive regimen instead. To validate the results against potential placebo effects, the company used ¹⁸F-DOPA PET scans, which showed increased tracer binding in the putamen at the injection sites, indicating cell survival and function.

Background & Context

The Phase I trial reported no dose-limiting toxicities, no serious adverse events, and no graft-induced dyskinesia. MRI scans showed no tumors or abnormal overgrowth. XellSmart has now finished enrolling its 30-person Phase II trial in China, utilizing a PROBE design where outcome scorers are blinded to the treatment group. This trial is led by Beijing Tiantan Hospital, with sub-centers at Peking Union Medical College Hospital, the Second Affiliated Hospital of Soochow University, and Jiangsu Province Hospital.

Significance & Outlook

XS411 received funding under China’s National Major Innovative Drug Program in June 2026 and FDA Fast Track Designation in July 2026. By avoiding personalized cell production and the need for “super-donor” HLA lines, XellSmart aims to solve the scalability and logistics challenges common in cell therapy. The upcoming Phase II control-group data will be the primary determinant of whether the five-hour gain is statistically significant.
